1. For schools and campuses, this application requires faculty/advisor/staff approval. 
2. The organization must be willing and capable to host, schedule, and moderate the presentation. For virtual presentations, Zoom is preferable but related video platforms are accepted. 
3. The school must confirm a scheduled booking (Virtual or In-Person) at a minimum of 4 weeks prior to the preferred event date.

4. Evaluation data must be collected from audience members after the program and returned to Active Minds. A host and audience survey will be provided by Active Minds.
